11/16/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah and her mother upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C engaged Ashiyah in a discussion to determine any new behaviors since the last session. Ashiyah reported there were no new behaviors to report since the last session besides the same thing Ashiyah continues to engage in and that is being argumentative when she cannot have her way with behaviors lasting 30 minutes in duration. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C during the session. she becomes upset she struggles with remaining calm and calming herself down as she will not pay attention to what is being discussed with her with behaviors lasting anywhere from 0-30 minutes in duration. 1. Intervention: QMHP-C inquired about Ashiyahs emotional well-being. QMHP-C recapped from previous session. QMHP-C informed Ashiyah that the session would focus on building positive interpersonal skills as reports from Ms. Hodge revealed that her negative interactions with peers are returning. Ashiyah role-played by modeling effective pro-social behaviors that she could demonstrate to prevent escalating and argumentative behaviors with peers, authority, and in the home. QMHP-C practiced effective assertiveness skills to teach Ashiyah to express feelings and thoughts in a way were everyone can respect and understand. 2. Intervention: QMHP-C taught Ashiyah utilizing coping skills to support with problem solving, engaging in age-appropriate decision-making, and conflict resolutions. QMHP-C encouraged Ashiyah to express feelings of anger and frustrations in a more constructive manner by thinking before acting and being more responsible for her actions. 1. Response: Ashiyah was receptive to QMHP-C's suggestions. Ashiyah verbalized that sometimes it can be difficult to make appropriate decisions as on many occasions she can be provoked into verbal aggression which can lead to physical altercations. Through role-playing and modeling Ashiyah was able to recognize her mistakes when interacting with peers and identified areas of improvement. 2. Response: Ashiyah verbalized during the session that when she becomes upset she struggles with remaining calm and calming herself down. Ashiyah revealed that she didn't want to consider herself as a punk when faced with confrontation. QMHP-C Interpretation of client’s progress towards today’s objectives: Ashiyah continues to struggle with being able to express her feelings and emotions in an assertive manner. QMHP-C Plan of Action: VACCD staff will continue to meet with Ashiyah to focus on her relationships with her peers and siblings in the home, school and community.
